CVMH is committed to providing your pet with a safe and healthy environment. To do so, we require written verification of vaccinations from a licensed veterinarian.
To expedite the check-in process, we strongly recommend all medical records be submitted prior to your pet’s arrival. The vaccine requirements below apply to any cat or dog visiting with us:
DAP (Distemper/Adenovirus/Parvo)-Required Annually or Annual Titer
Rabies – Within label requirements (every 1 or 3 years)
Bordetella – Required Annually
Intestinal Parasite Exam – Required Annually
